So is our name based off the forum/idenity account or do you just make up a roleplay name in the server? Like John Prickle

If I may friend, I would assume that you would make one up in the server. Per say my Roleplay name is "Quansian D. Attachi" while my forum name is QuannTann.

It will be a First.Surname format most likely by a character basis e.g. every character you make, you will choose the name in that format.
